It’s important to know this before you go so your Physical Therapist can put together a plan that can be completed before you run out of visits!

Do It Yourself: Steps YOU Can Take

To find out what your benefits are you will have to:

-Call your insurance company (the number is on your health insurance card)

-Wait until you can speak to an actual person (you may have to wait for 30-90 minutes)

-Know what questions to ask

Put your full name, social security number and your phone number on the fax coversheet.

An easy way to do this is to take a picture with your phone and attach the images to the email. Put your full name, social security number and your phone number in the email.
